- Facility location: the street address, city, service entrance and affected bay or door.
- Equipment and symptom: what has stopped working, visible impact damage, and whether loading or building security is affected.
- Site contact: who can provide access, authorize work and supply any purchase order or vendor-onboarding requirements.
A brand, model or photo may help later. You do not need to collect photos or complete a form before calling.
Calling from outside Indianapolis?

For multi-building properties, identify the building and affected bay. A postal city can differ from the municipality where the facility is located, so the street address is more useful than a city name alone.
For planned loading dock maintenance, give the number of doors, levelers, and other dock components to include. For a repair, say whether the door, the leveler, a portable plate or board, or several components are affected. You can call before collecting photos; only take them from a safe location.
Urgent or scheduled work
Say immediately if a bay is unusable, a door is unsecured or damaged equipment is blocking operations. For scheduled repairs or preventive maintenance, give the equipment count and an access window.
